Southern Forests Medical Centre offers GP Management Plans and Team Care Arrangements for our patients.  These are written management plans to assist with the management of chronic medical conditions such as diabetes, asthma, arthritis, cancer, heart disease and osteoporosis.  The plan is made with you and your doctor to identify health priorities and discuss benefits that you may receive from other health care providers or allied health professionals.  Having a plan allows you to 5 visits per calendar year to see allied health professionals.
